FCRA Consent Forms
Before submitting a background report order through our website, you must obtain written authorization from the applicant. These forms are available under the Docs and Forms menu after you log in.
Order Reports
Option 1 - Applicant Invitation
Description: The CRC platform allows for client users to send applicants a link to the applicant driven portal. This allows your team to securely collect the candidates Personal Identifying Information and authorization for the report while eliminating the burden of data entry.
- From the menu, locate the "Applicant Quick Invite" tile.
- The client user enters the candidate’s first name, last name, and email address into the Applicant quick invite tab.
- Select the package you'd like to use for the order.
- Select to either "Save to Draft for Review" or "Process Immediately". "Process Immediately" is the default option which will put the order in production as soon as the candidate finishes their side of the process.
- Click the "Send to Applicant" button. With that, the user has finished their part of the submission process.
Option 2 - Online Order
- Click on “Ordering” from the menu.
- You will see an order form appear.
- Select the report components you would like to order.
- Fill in the Subject Information section.
- Click on any red tabs below the Subject Information section to finish providing us the information required to perform the search.
- Click on the checkbox in “Section 4: Authorize” to agree to the certifications and agreements listed below and then click Continue To Next Step - Verify Order to proceed to submit the background check.
- If you have provided all the information required, all buttons will be green. If not, please click the red “Describe Errors” button to see the additional information needed to perform the search.
- Click on the “Submit Order” button to request the report.
- A verification screen will appear, indicating which reports you ordered. If the information is correct, press “Submit Now” and a confirmation screen will appear. If any information is incorrect, hit the “Go Back” button, correct the data, and resubmit.
Option 3 - Quickscreen
- From the Home page, enter your applicant SSN, Select Package and FCRA purpose, and then press the “Submit” button.
- Complete the form, and then select the “Continue to next step” button.
- Select any additional report components you would like to order.
- 4. Fill in the Subject Information section and select the green “Continue to next step” button.
- Refer to #9 in Option 1.
View Reports
- To retrieve your results, click on “Checked Individuals” from the menu.
- This displays an alphabetical listing of the last 50 candidates’ results. Click on the name of the applicant to retrieve results.
- For additional report viewing options, click on the “Filter” button at the top of the screen.
Please note: If you decide not to hire an individual or take other adverse action based on information from our report, you must follow the proper procedures under the Fair Credit Reporting Act (FCRA).
